# End-Term Evaluation Process Not Initiating After Mid-Term Form Completion
### Issue
Despite completing the mid-term evaluation form, the end-term evaluation process fails to commence.
### Solution:
The end-term evaluation form will be generated once all parties, including the supervisor and teacher, have completed and signed the mid-term evaluation form.
