The Transfer Interview Form serves as a crucial tool in assessing a student's progression during practical training or internships. It can be generated through two distinct methods:
Automatic Generation:
If a Possible Failure in Practice form is created during the midterm evaluation or final evaluation phase, the Transfer Interview Form is automatically generated. This ensures that any potential concerns about a student's performance are addressed comprehensively.
Independent Generation:
In cases where the Possible Failure in Practice form hasn't been generated during the midterm or final evaluation phase, the Transfer Interview Form can be independently generated during the final evaluation. This allows for a dedicated assessment during the culmination of the practical training or internship experience.
To create the Transfer Interview Form, check the checkbox next to 'Need for Transfer Interview' and confirm your selection.
Filling out the form
Accessing the Form: Click on the form name next to the 'Possible Failure Form' on the 'Evaluation Phases' field at the top.
Filling Out Necessary Fields: Ensure all required fields are completed.
Reason for Creating the Form for the Student: Clearly articulate the reasons for initiating the Transfer Interview Form.
Sending a Copy to the Student: Check the checkbox if the student will receive a copy of the form. This provides transparency and ensures the student is aware of the assessment and recommendations.
